Incident Narrative

An employee was checking the taped hole he created on a shaft and was pulling the tail stock back on the lathe. The employee leaned over the machine and the lead screw caught his shirt, pulling him into the machine. As it pulled tighter, he was forced toward the machine, impacting his right shoulder. The employee suffered multiple fractures and torn tendons to right shoulder.
