Incident Narrative

A temporary employee was on top of a conveyor servicing a robot contained within a safety cage. A sensor at the end of the conveyor was engaged, which initiated an auto cycle. The robot contacted the employee, causing him to go in the air and strike the safety cage. The employee suffered a skull fracture. The robots were guarded but not locked out/ tagged out at the time of the incident.
