Incident Narrative
On May 24, 2023, an employee was on a 20-foot ladder running a test on the control panel at the top of a transformer. The employee began descending the ladder and reached the third or fourth rung, when the ladder shifted, causing the employee to fall with the ladder. The employee's right leg was caught between the rungs resulting in a right ankle fracture.
