Incident Narrative

An employee was in a mobile oil exchange trailer, climbing over an oil tank to adjust an oil hose reel. The tank valve opened, causing the pressurized tank fill cap to strike the employee in the face. The employee lost multiple teeth and suffered lacerations and fractures to the face.
