Incident Narrative

A crew was removing bolts from the pump's spool piece and rigging the hoist to hold the elevated spool piece out. The crew replaced the witch's hat inside the pump and began to reinstall the spool piece when a burst of fire came out of the pipe and struck the injured employee's face and hand resulting in burns.
