Incident Narrative

An employee was moving a metal container of defective, water-filled bottles (which were about 700 degrees Fahrenheit) when a flash fire occurred. This probably occurred because one of the hot bottles fell to the ground and made contact with machine oil residue. The employee suffered a second degree burn to his right cheek and left ear from the heat of the flash fire, requiring hospitalization.
