Incident Narrative

An employee was changing a tire on a dual-axle tow truck. The employee drove the truck onto a piece of block and removed the outer tire in order to access the inner tire. When putting the outer tire back on and shaking the truck, the inner tire fell off the piece of wood, causing the outer tire to knock him to the ground. The employee sustained a femur fracture and was hospitalized.
