Incident Narrative

An employee was pulling a pipe from a rack using a tool when the tool slipped, causing her to lose her balance in a backwards motion. While in motion, her foot became caught on a rail (extends approximately 1/2-inch above the surface), causing her to fall on and break her right hip in two places. She was hospitalized.
