Incident Narrative

An employee had been removing baffle plates from a tower in a facility. While moving to an upper level on a scaffold, he lost his footing and fell approximately 8-9 feet to a lower level and impacted a handrail on scaffolding below. The employee sustained five fractured ribs on the right side and internal bleeding.
