Incident Narrative

On December 22, 2021, employees were using aerial lifts to install ceiling liner and panels. An employee was working from a raised aerial lift and leaned against the lift's door gate, then fell 8-10 feet to the dirt, gravel and snow-covered ground below. The employee was hospitalized with fractures to their L2 and L4 vertebrae and lung lacerations. Fall protection was not attached at the time.
