Incident Narrative
An employee was working on wear strips for a silo safety gate at an asphalt plant. As the employee was moving the gate with a pry bar, it slid off its rollers and landed on the employee's hand. The employee suffered one finger amputation down to the middle knuckle, two finger amputations just below the fingernail, and a broken thumb.
