Incident Narrative
An employee removed plywood that was covering a hole so that a crane could fly in a flange. The employee fell through the hole in the floor, contacting a flange approximately 41 inches below. The employee then fell approximately 47 inches, contacting an aluminum step stool, before falling approximately 19 inches to the ground. The employee was hospitalized with a spinal injury that required surgery.
