Incident Narrative

An employee was removing plastic shards from a machine using a compressed air hose. She stepped on the guardrail to gain better access to the accumulated plastic and her hair was caught in the rotating canopy drive shaft. The employee suffered hair loss and scalp damage. The machine was not guarded at the time.
