Incident Narrative

An employee was welding on a flange that weighed over 1,000 pounds. The flange fell from the positioner it was on and struck the employee, causing him to fall to the ground level. The employee's leg was caught underneath the flange, resulting in a below-the-knee amputation of the left leg.
