Incident Narrative

An employee was trimming a 2 x 4 piece of wood using a table saw. As the employee was guiding the push stick, the wood kicked back and his left hand slipped. His middle finger was then caught on the blade, resulting in a partial amputation of the fingertip and a laceration to the right ring finger.
