Incident Narrative
An employee was removing a 300-400-pound valve from a wellhead using a crane. The auto crane held tension on the valve and a bolt got hung up between the two flanges. When the bolt came free, the valve dropped and landed on the employee's finger. It crushed his finger against the wellhead flange, resulting in a laceration and broken right ring finger.
