Incident Narrative

A crew was placing spacers on a new water pipe that was lifted on one end to help with the placement of the spacers. The strap began to twist and the pipe struck the employee's right foot, resulting in a laceration to the right big toe and a foot fracture. The employee was hospitalized and required surgery.
