Incident Narrative

An employee was moving an overhead crane and had the crane remote in his hand. He was lifting a bundle of rebar with the crane. There was another free-swinging chain hanging from the crane. The hook at the end of the free-swinging chain hit the employee in the face, crushing the right side.
