Incident Narrative

An employee was preparing to remove concrete barriers from molds using a spreader bar that would be attached to a crane. The rigging attached to the spreader bar became caught on a small rebar hook that was welded to the center of the spreader bar. When the employee began to move the spreader bar, it fell sideways, striking the employee and causing a closed fracture of the right tibia.
