Incident Narrative
An employee was repairing a forklift. The vise chain had been placed on the outer core barrel while the inner core barrel was being removed. The outer core barrel was moved and the employee's left hand was caught between the handle of a chain vise and the fork of the forklift, resulting in amputation of the left ring finger above the knuckle.
