Incident Narrative

On January 23, 2023, an employee was performing maintenance on a generator in the generator room. As the employee was leaving the area, gas emitting from the generator was ignited by a diesel powered heater. The employee suffered first- and second-degree burns to the back, face, and hands. The valves were not locked out or tagged out at the time.
