Incident Narrative

On January 10, 2023, a crew was excavating a ditch to place pipes. An employee brought the road plate to the ditch and as he was lowering it, one side of the plate began to shift. The injured employee helped manually guide the plate. Then, the side opposite of where the injured employee was standing, began to move out of place. Upon going to move it, an excavator also went to move the plate with the bucket, pinning the employee between the excavator bucket and steel plate. The employee sustained neck fractures.
