Incident Narrative
An employee was matching redundant parts in a CNC lathe. A part came out of the chuck, hitting and breaking the guard door glass. The glass struck the employee in the left side of his face, rendering him unconscious and causing him to fall to the floor. The employee suffered skull, orbital, and nasal fractures.
