Incident Narrative

An employee was unloading boxes from a service vehicle to place onto carts. When he finished, he began collapsing a cart. The employee put his right foot on the cart to steady it but it began rolling. The employee then slipped and fell, striking his head on a cart; he sustained a scalp laceration.
