An employee was on a ladder accessing the top of a vessel. He was using a hand wrench to tighten an outlet piping flange. The wrench slipped off the flange, causing the employee to fall 8 feet to the ground and sustain 4 rib and 3 back fractures. Fall protection was not worn at the time.
