Incident Narrative

An employee was working on an overhead garage door opener, reprogramming the controller. He was approximately 5 to 6 feet up on a 10-foot A-frame ladder when the door was opened. He reached for the door and lost his balance on the ladder, falling to the concrete floor. He suffered a broken right tibia and fibula and a right shoulder rotator injury.
