Incident Narrative
Employees were in an excavation installing a 36-inch pipe inside of a 7-foot diameter circular structure. The pipe was suspended by a weight bearing cable attached to a trackhoe. As the pipe was guided in, it was not setting properly and was removed. The pipe became dislodged from the structure and swung backward, striking the injured employee in the chest. The employee was hospitalized for a chest laceration and tingling in the arm.
