Incident Narrative

On October 8, 2019, at approximately 9:54 a.m., an employee was removing a broken section of concrete sidewalk from around a wrought iron fence. As the employee manually lifted and moved a section of concrete away from the fence, the employee was struck in the face by the bucket of an excavator. The employee's head was temporally pinned between the bucket of the excavator and the fence. The employee was hospitalized for treatment of a broken arm, broken jaw, separation of the right ear, and injuries to the head and neck.
