Incident Narrative

An employee was taking a chemical sample from a tanker truck. The 12-foot high catwalk retracted while the employee was taking a sample and they fell backward off the truck. The employee struck several objects on the way down and sustained a hip and vertebrae fractures as well as a punctured kidney.
