Incident Narrative

A forklift operator was picking up a cylindrical container filled with brass shavings and dumping it into a container. The overflow was being swept up by the injured employee and put into a cardboard box. The cylinder rolled off the forklift forks and flipped over onto the injured employee resulting in broken bones in the back and a possible concussion.
