Incident Narrative

An employee was standing on a ledge of a precast T-girder using a trowel with mortar to fill recesses in the beam. The employee fell approximately three feet, six inches from the ledge to the ground, striking a concrete block and possibly the adjacent girder that was located behind him. He sustained fractures of the right fifth through ninth ribs, a contusion to the forearm, abrasion and contusion to the face, and a laceration to the tongue and was hospitalized.
