Incident Narrative
An employee was prepping a hopper that was being welded. An auger was still in operation under the hopper and metal plates were placed onto the auger to secure it. The employee was placing fire blankets down when one of the plates shifted and created an opening to the auger. The fire blanket that the employee was standing on was caught in the auger, pulling the employee into it. The employee was hospitalized and their left leg was amputated.
