Incident Narrative
An employee was repairing a hole punching assembly. He used a small machinist hammer to free the boost cylinder, then lubricated the shaft and tested the cylinder. As he was retrieving the machinist hammer at the base of the punch, the roll forming machine activated and the punch struck his right ring finger. He suffered a fingertip amputation.
