Incident Narrative

An employee opened a hotdog packaging machine to clear a film jam in the machine. The employee restarted the machine after clearing the jam, but the film began to jam again. The employee attempted to clear the jam again when their left index and middle fingers contacted the shear between the sealing bucket and the sealing head, resulting in partial amputations of the fingers to the bottom of the nail beds.
