Incident Narrative

An employee was lowering his tools to the ground while on top of a roof at a job site. The tools were on a pallet with a strap attached to it. When the employee tossed the pallet off the roof, the strap caught his foot and leg, pulling him off the roof to the ground below. The employee suffered broken bones.
