Incident Narrative

An employee was working from a baker scaffold. The wheels of the scaffold went into a section of concrete that had not been poured yet, causing the scaffold to tip and the employee to fall. He reached out and his left arm was caught on a metal track, resulting in a laceration that required hospitalization.
