Incident Narrative
An employee was setting floor joists on a mezzanine that was being constructed inside a lumber warehouse. While transitioning to an eight-foot step ladder, the employee fell 12 feet, landing on the concrete floor below. They were hospitalized for head trauma, pulled tendons in the left shoulder, a shoulder dislocation, and torn ligaments. Fall protection was not worn at the time.
