Incident Narrative
An employee was installing TPO roofing on the parking garage side of a 3-story building. When he lifted a sheet of insulation, the wind pushed the insulation and caused the employee to lose balance and fall 10 to 12 feet onto the third floor level of the parking garage. The employee sustained injuries to the ribs and arm. The employee was wearing a lifeline at the time.
