Incident Narrative
On the morning of July 21, 2022, two employees were at a stacked rig, removing 4-inch high-pressure piping. They began to break the nuts loose on the saddle clamp bolts under the valve tree. As they were breaking loose the last nut, the piping suddenly rotated in the saddle clamp and the transducer valve tree (about 4 feet tall, weighing 250 pounds) pivoted 90 degrees toward the ground and landed on one of the employees. The hammer union ears at the top of the valve tree struck him in the middle of the shoulder and back, causing contusions.
