Incident Narrative

An employee was climbing down a 5-foot-tall masonry scaffold using the ladder on the side. His foot slipped on the rung and he fell backward from the ladder, landing on 1-inch PVC electrical conduits stubbed up out of the ground. A conduit broke a thoracic vertebra.
