Incident Narrative
While setting up a plate saw for production, an employee pressed a button to move one table hydraulically toward the other table on a glide wheel, but the hydraulics did not work. The employee was moving the table by hand when their left index finger was pinched between the two tables, resulting in an amputation above the first knuckle.
