Incident Narrative

Maintenance was being performed on a machine that inverts coal cars to dump product out of the cars. The machine was in the inverted position at the time. An employee was standing on fiberglass grating when it came loose causing them to fall 21 feet to the surface below. The employee sustained a broken arm. The employee was wearing fall protection, but it was not connected at the time of the fall.
