Incident Narrative
An employee was using a forklift to place material on the third tier of a warehouse rack when the forklift struck a ceiling beam and the resulting force discharged the employee from the forklift. The forklift then tipped over onto the employee, pinning him until other employees could extricate him. The employee suffered blunt force trauma to the spine, dislocation of vertebrae, and incomplete paralysis of the torso and lower limbs.
