Incident Narrative

A bottle became stuck on the empty bottle conveyor line. Two employees approached the bottle from opposite directions and arrived at the bottle at the same time. The section of platform the employees were standing on failed and the employees fell 9 feet to the floor below. One employee was treated and released. The second employee was hospitalized for a C7 fracture, a T1 transverse fracture, a T12 chance fracture, and a L1 superior end plate compression fracture. Her injuries required surgery.
