Incident Narrative
A manual, chain-operated dock leveler was pinched between a trailer and a dock, partially raised. Two employees were trying to lift it. When the leveler cleared the trailer, its pivoting, hinged lip sprung open and pinched one employee's right thumb against the fixed edge of the leveler. He suffered an amputation to the thumb.
