Incident Narrative

On April 24, 2022, an employee was standing on the top step of an 8-foot A-frame ladder, installing the cover of a roll-up gate. The ladder tipped over and he fell to the concrete ground, suffering a torn knee ligament. He was hospitalized and underwent surgery.
