An employee was installing a chute cover (weighing approx. 200-250 pounds) onto a chute, using a come-a-long to lift the cover. A lifting lug on the chute cover broke causing the cover to fall and strike the employee on the left shoulder. The employee sustained a broken collarbone, requiring hospitalization and surgery.
