Incident Narrative

An employee was moving a section of glass from a motorized pallet jack into a vertical washing and inspection machine. The jack moved unexpectedly and the section of glass fell off the pallet and pinned the employee against the washing and inspection machine. The employee was hospitalized with fractured ribs.
