Incident Narrative
An employee was checking for air leaks in a vacuum system after the vacuum pump had been removed from the airlock rotary valve. The employee slipped on water, fell to one knee, and grabbed the airlock to steady himself. His left middle finger contacted the moving vanes inside the rotary valve, causing a fingertip amputation. He was hospitalized. The airlock was not locked out at the time of the incident.
