Incident Narrative
An employee was installing a pipe into a placing boom. The pipe was suspended by a tower crane with a nylon strap using the hitch and a half hitch rigging method. The pipe slipped in the rigging and dropped 3 to 4 inches, crushing the employee's right thumb and little finger. The thumb was amputated and the little finger was severely lacerated.
